Baby Dedication Form
Thanks for contacting us regarding dedicating your child to God. This is an exciting ceremony that includes a
prayer of blessing for the child and their family, the commitment of the parent(s) to raise their child in God’s
ways, but also the commitment of our church to do our part in assisting you as the parent(s) in this very
important matter.
We want to make it very clear that this dedication ceremony does not ensure your child’s salvation as understood
within Christianity. In order for an assurance of salvation, your child must personally put their faith in Jesus
Christ as their Lord and Saviour. For this dedication, you will be asked to agree to do your part to raise your child
in an atmosphere where they might naturally want to come to a personal profession of faith in Jesus Christ.
During the ceremony, you may be dedicating your child along with other parents\families. A pastor will talk to
all of the families involved on the day of the ceremony, as well as bring attention to you and your child during
that moment, praying over your child/family and the other children/families involved. During the course of the
group talk, the following two questions will be asked of you…
• Do you now present your child to God in solemn dedication?
• Do you profess today, as their parents, that while relying on God’s grace, you will seek to bring him/her
up in the values of God’s Kingdom, so that they follow Jesus Christ and make Him central for life?
If you agree to these questions, please check the appropriate box on the application form, fill in the rest of
the fields, and send it back to the church office ASAP. You will be contacted with the next available date for a dedication ceremony.
We are glad to be able to celebrate this great day with you and we trust that you will join us regularly and give
us a chance to do our part in assisting you to raise your child in God’s ways.
